Transition Navigator
Midland, MI

Full-time Transition Navigator s primary duties include working with individuals and families in need of a wide variety of assistance including community resources and referrals to area organizations. Responsibilities include providing information about supports for older adults and caregivers, making referrals to community resources, and conducting intake assessments for long-term care programs available through that allow adults with complex medical needs to live at home.

Responsibilities of a Transition Navigator:
Provide direct service to Medicaid recipients residing in nursing facilities, who desire to live in the community.

Assess the needs of consumers
Arrange for, schedule, coordinate and assist with consumer transitions.
Provide follow up services, including coordination and support.
Collaborate with Nursing Facilities and other Agencies to facilitate the consumer s move to the community
Provide information and referral services as requested.
Conduct intake assessments for long-term care programs available
Ability to drive and travel to residential appointments and activities in all counties of the region.
Assist with development of the person-centered plan of care.

Schedule of a Transition Navigator:

Monday-Friday 8:00 am - 4:30 pm

Compensation of a Transition Navigator:

$15/hr

Job Requirements

Requirements of a Transition Navigator:

At least three years of experience in the provision of health and social services
A bachelor s degree in a health or human services field or Community Health Worker certification desired
Experience working with complex older adults is desired
Knowledgeable with person-centered planning
Excellent interpersonal communication skills.
Ability to initiate and maintain positive working relationships with staff, clients, and other organizations.
Must be accurate, conscientious and flexible with attention to detail and is proficient with computers.
Ability to communicate with elderly or disabled participants on a one-to-one basis
